Job Summary
Conduct comprehensive in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests such as spirometry or diabetic eye exams. Perform virtual visits as needed while utilizing an iPad-based clinical workflow to connect with patients in their private residences. Provide all specified Signify Health services, including preventive screenings, while maintaining flexible travel across licensed states and overnight stays. Engage in one-on-one conversations to address patient concerns and guide them toward appropriate care without prescribing medication or altering existing treatment regimens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
