Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad clinical workflow to connect one-on-one with patients, reviewing medical history and answering health questions without prescribing medication or altering treatment regimens. Travel up to 100% of the time across defined coverage areas, potentially requiring overnight stays or flights, while maintaining active, unrestricted multi-state licenses. Perform duties in diverse residential environments, including homes with pets or allergens, and conduct virtual visits as needed. This role supports Signify Health's mission to bridge care gaps nationwide by providing personalized, comprehensive preventive and diagnostic services directly to patients' front doors.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
