Job Summary
Conduct in-home health evaluations across Maine, performing 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad for clinical workflows to connect with patients, review medical histories, and provide education without prescribing or altering treatment regimens. Travel up to 100% of the time, including overnight stays, to cover all counties within Maine and flex to other areas as needed. Perform virtual visits when required and receive per-completion incentives for each evaluation. Maintain active, unrestricted multi-state licenses and board certification to fulfill role requirements.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover the following primary locations: Able to cover all counties within Maine and flex to other areas of business need
- Maine Coverage Markets include: Aroostook, Androscoggin, Kennebec, Knox, Lincoln, Sagadahoc, Waldo, Franklin, Oxford, Somerset, Hancock, Penobscot, Piscataquis, Washington, Cumberland and York
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
