Job Summary
Conduct 45-minute in-home health evaluations across defined coverage areas, including medication reviews, physical assessments, and diagnostic services like spirometry and diabetic eye exams. Perform virtual visits as needed while utilizing an iPad-based clinical workflow. Maintain active, unrestricted multi-state licenses to support travel and overnight stays within licensed states. Deliver preventive and diagnostic services directly to patients in their residences, adhering to safety protocols for diverse home environments.

- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
