Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform services such as bone density screening while connecting one-on-one with patients. Travel up to 100% of the time across a defined coverage area in Oregon, which may involve overnight stays or flights, and perform virtual visits as needed. Maintain active, unrestricted multi-state licenses and board certification to fulfill role requirements. Work full-time hours with a focus on building trusted relationships and guiding patients toward better health outcomes.
