Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, performing medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams. Deliver comprehensive preventive and diagnostic services directly to patients' residences, utilizing an iPad for clinical workflows. Fulfill requirements as a clinician colleague within Signify Health's national network, conducting virtual visits as needed. Maintain active, unrestricted multi-state licenses and board certification to cover all counties within Maine and flex to other areas of business need. Work full-time hours with up to 100% travel, including overnight stays and flights, while receiving per-completion incentive pay.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within [state] and flex to other areas of business need
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
