Job Summary
Conduct in-home health evaluations across New Hampshire, performing 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad-based clinical workflow to connect with patients in their private residences, addressing health questions and guiding care without prescribing medication or altering treatment regimens. Travel locally and nationally up to 100% of the time, including overnight stays and flights, while maintaining active, unrestricted multi-state licenses and board certification. Perform virtual visits as needed and receive per-completion incentives for all completed evaluations.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within New Hampshire and flex to other areas of business need
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
