Job Summary
In-home Traveler Nurse Practitioner or Physician Assistant (Full-Time) provides in-home health evaluations to patients in their residences, traveling up to 100% within a defined coverage area (New Hampshire). Role includes 45-minute visits for medication and medical history review, physical examination, and optional diagnostic tests; uses an iPad and a clinical workflow; may include virtual visits; must hold active licenses and board certification, with ability to obtain multi-state licenses. Primary work environments are patients' homes, with flexibility for overnight stays, travel logistics, and potential coverage across multiple counties in NH. Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (NP or PA)
- Able to obtain and maintain multi-state licenses in coverage area(s)
- Able to cover the following primary locations: Able to cover all counties within New Hampshire and flex to other areas of business need
- Travel up to 100% across defined coverage area as required
- In-home health evaluations experience (preferred)
- Nurse Practitioner or Physician Assistant credentials
