Job Summary
Conduct in-home health evaluations across Massachusetts, performing 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad-based clinical workflow to deliver preventive services while connecting one-on-one with patients in their private residences. Travel up to 100% of the time within licensed states, potentially requiring overnight stays, and perform virtual visits as needed. Maintain active, unrestricted multi-state licenses and board certification to fulfill requirements for covering all counties in the defined coverage area.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Massachusetts and flex to other areas of business need
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
