Job Summary
Conduct in-home health evaluations across Massachusetts, performing medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams during 45-minute visits. Utilize an iPad clinical workflow for each appointment while maintaining flexible travel schedules up to 100% of the time, including overnight stays and flights as needed. Perform virtual visits when required and provide comprehensive preventive services directly at patient residences. Earn additional per-completion incentive pay for all completed visits. Maintain active, unrestricted multi-state licenses and board certification to cover all counties within the defined area.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Massachusetts and flex to other areas of business need
- Flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
