Job Summary
In-home Health Evaluations conducted by a Nurse Practitioner or Physician Assistant traveling across Massachusetts. The role travels up to 100% of the time, visiting patients in their homes to perform med-history review, 45-minute physical evaluations, and any ordered diagnostic tests (e.g., spirometry) as part of individualized health assessments. The clinician uses an iPad and a clinical workflow, does not prescribe medications or alter current treatment plans, and may conduct virtual visits as needed. Requires active, unrestricted licenses and board certification with the ability to obtain/maintain multi-state licensure to cover MA and other business needs. Sign-on bonus available. Full-time position with benefits (health/dental/vision, PTO, 401K, CEU allowance, malpractice coverage). Travel may involve overnight stays and occasional flights, depending on the defined coverage area. The typical weekly hours are 40. The role is based in Massachusetts with a primary residence requirement in MA. Locations include MA counties such as Barnstable, Dukes, Nantucket, Bristol, Norfolk, Plymouth, Suffolk, Essex, Middlesex, Berkshire, Franklin, Hampden, Hampshire, and Worcester.

- Able to cover the following primary locations: Able to cover all counties within Massachusetts and flex to other areas of business need
