Job Summary
Conduct in-home health evaluations across Maine and surrounding markets, visiting patients in their private residences for 45-minute sessions that include medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad-based clinical workflow to perform these assessments while adhering to strict safety protocols in diverse home environments that may contain pets or allergens. Travel up to 100% of the time, including overnight stays and flights as needed, to cover all counties within Maine and flex to other business areas. Perform both in-person and virtual visits as required, focusing on patient education and care coordination without prescribing medication or altering existing treatment regimens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Maine and flex to other areas of business need
- Primary residence in the state of Maine
- Full-time employment status
- Able to travel up to 100% of the time
- Able to conduct overnight hotel stays, car rentals and/or flights
- Able to conduct virtual visits as needed
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
