Job Summary
Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations across New York and surrounding markets. Clinician will travel up to 100% within defined coverage areas, performing 45-minute visits including medication history, physical evaluation, and possible diagnostic tests. Requires active, unrestricted licenses, board certification, and ability to obtain multi-state licensure; primary workplaces are patient homes with travel to various counties in NY and beyond as needed. Sign-on bonus eligible. Benefits include health/dental/vision, PTO, 401K, CEU allowance, malpractice coverage. Travel logistics may involve overnight stays, car rentals, or flights; ideal candidate comfortable with home environments and diverse living conditions.
