Job Summary
Conduct in-home health evaluations across New York State, performing 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad-based clinical workflow to connect one-on-one with patients, reviewing medical histories and answering health questions without prescribing medication or altering treatment regimens. Travel locally and within licensed states up to 100% of the time, potentially requiring overnight stays and flights to cover multiple counties including Albany, Buffalo, NYC, and Rochester. Maintain active, unrestricted licenses and board certification while working as a clinician colleague within Signify Health's national network.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within New York and flex to other areas of business need
- New York State Coverage Areas Include: Albany, Binghamton, Buffalo, Elmira, NYC, Plattsburgh, Poughkeepsie, Rochester, Syracuse, Utica and Watertown
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
