Job Summary
Conduct comprehensive, home-based behavioral health assessments including the CANS tool and develop coordinated treatment plans for families. Provide intensive family therapy, skills training, and coaching for youth and caregivers while identifying and linking to community resources. Manage care coordination by attending IEP, hospital discharge, and CPT meetings, providing collateral contacts, and facilitating transportation as needed. Respond to phone crises, participate in on-call rotations, and complete all required documentation. Maintain working relationships with Community Service Agencies and community partners while actively participating in supervision and staff meetings.
Required Qualifications
- Master's level degree in social work, counseling or related field
- Certification to administer CANS-MA version
- Must complete in Home Therapy training program upon acceptance of position and prior to providing services
- Must be proficient in use of standardized documentation procedures
- Must be able to perform each essential duty satisfactorily
- Ability to communicate effectively verbally and in writing
- Ability to develop and maintain satisfactory working relationships with employees and providers
- High energy level, superior interpersonal skills and ability to function in a team atmosphere
- Commitment to Advocates values and mission
- Must hold a valid drivers' license
- Must have access to an operational and insured vehicle and be willing to use it to transport members
- Must be available at least 2 night per week until 8 pm
- Must be available for evening shifts (11-7pm or 12-8pm)

- Experience working with children/families and adolescents in child development, childhood mental illness, or family issues
