Job Summary
Consult with probation officers, school staff, and treatment teams regarding referrals and develop Individual Service Plans for clients referred to the Lorain Intensive Home Based Treatment program. Provide diagnostic assessments, counseling, crisis intervention, and community support services including biopsychosocial reviews and family therapy. Maintain caseloads and productivity levels while documenting clinical records, billing data, and termination reports in accordance with licensing standards. Provide 24/7 on-call coverage on a rotational basis and offer culturally competent individual and group interventions. Serve as the primary direct service clinician in home and community settings with no subordinates.
Required Qualifications
- Masters Degree in Social Work, or Counseling
- Valid Ohio LSW, LISW, LPC, or LPCC
