Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to deliver preventive and diagnostic services to patients in their private residences. Perform virtual visits as needed and travel locally within licensed states to meet health plan members. Maintain flexibility for on-call rotations and adhere to a part-time schedule of approximately 12 weekly hours. Engage in one-on-one conversations to build trusted relationships and bridge care gaps for individuals seeking personalized health support.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
