Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to connect one-on-one with patients in their private residences, reviewing medical history and answering health questions without prescribing or altering treatment regimens. Maintain flexibility to travel locally across licensed states and perform virtual visits as needed. Work part-time hours with a base hourly rate plus visit-based incentives, while providing all Signify Health services including preventive screenings. Candidates must be comfortable performing duties in diverse home environments that may contain pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
