Job Summary
Per diem physician to provide in-home health evaluations in patients' homes, traveling locally within licensed states. Role includes 45-minute visits with one-on-one health discussions, medication/history reviews, physical evaluations, optional diagnostic tests, and use of an iPad with a clinical workflow. No prescribing, lab orders, or treatment changes required. Part-time position with malpractice insurance coverage during Signify Health activities and benefits eligibility. Location: Burlington, IA; travel within licensed states as needed. Requires active MD/DO license(s) and ability to conduct in-home visits with flexibility for travel; compensation includes hourly base rate with incentive structure per visit.
