Job Summary
Provide in-home health evaluations as a Part-Time Physician for Signify Health (CVS Health). Conduct ~45-minute visits delivering one-on-one health discussions, medication and medical history reviews, physical evaluations, and, if requested by the health plan, diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Utilize an iPad-based clinical workflow; no prescribing, lab orders, or treatment changes required during visits. Travel locally within licensed states to member homes; virtual visits may be conducted as needed. Collaborate within Signify Health’s clinician network to connect members to appropriate care, while maintaining patient-centered care in the home. Prioritize safety, quality, and compassionate engagement in diverse home environments and be flexible to travel as needed.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MD or DO degree
- Multi-state licenses (preferred)
- Anticipated Weekly Hours 29.5 (Part-time)
