Job Summary
In-Home Physician role for Signify Health (CVS Health) in Trenton, NJ, part-time. Conduct ~45-minute in-home health evaluations, including medication and medical history review, physical examination, and optional diagnostic tests (e.g., spirometry, diabetic eye exams) as ordered by the health plan. Use an iPad and a streamlined clinical workflow; not required to prescribe medications, order labs, or change existing treatment plans. Travel locally to patient homes and provide both in-home and occasional virtual visits as needed. Active MD/DO licenses, malpractice coverage, and ability to work within licensed states are essential. Hours ~29.5 per week with benefits for part-time staff.
Required Qualifications
- MD or DO with active, unrestricted license
- Multi-state licenses preferred
- Part-time availability
- Ability to perform in-home health evaluations including history, physicals, and applicable ancillary testing

- Ability to travel locally to patient homes within licensed states
