Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these services while reviewing patient homes for accessibility factors. Engage one-on-one with patients in their residences to discuss health journeys, answer questions, and facilitate referrals to appropriate care teams. Travel locally and within licensed states as needed, and conduct virtual visits when required. Work part-time hours totaling 29.5 weekly. This role supports Signify Health's mission to bridge care gaps and provide personalized, compassionate health experiences nationwide.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
