Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to provide preventive and diagnostic services while reviewing medical history. Perform virtual visits as needed and travel locally within licensed states to reach patients in their homes. Engage one-on-one with individuals to bridge care gaps and connect them with appropriate resources.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
