Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests such as spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform services including preventive screenings while maintaining patient privacy in diverse home environments. Work locally and virtually as needed, engaging one-on-one with patients to bridge care gaps and facilitate referrals. Requires active, unrestricted medical license; part-time schedule of 29.5 hours weekly with hourly compensation ranging from $83.69 to $180.25. Application deadline is December 31, 2026.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
