Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to provide comprehensive preventive services without prescribing medication or altering treatment regimens. Travel locally within licensed states as needed to visit patients in their private residences, adapting to diverse home environments that may contain pets or allergens. Maintain active, unrestricted medical licenses in the coverage area while supporting Signify Health's mission to bridge care gaps and improve patient outcomes nationwide.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
