Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform services such as bone density screening while reviewing medical history. Travel locally and conduct virtual visits as needed, adapting to diverse home environments that may contain pets or allergens. Maintain unrestricted medical licenses and work part-time hours of 29.5 weekly. This role connects patients directly with care through Signify Health, a division of CVS Health, offering personalized assessments to bridge gaps in traditional healthcare.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
