Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform preventive services such as bone density screening while reviewing medical history. Engage one-on-one with patients to answer health questions and connect them to appropriate care plans. Travel locally to visit patients in their private residences, adapting to diverse home conditions and potential allergens. Perform virtual visits as needed and collaborate with Signify Health's national network to improve patient outcomes.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
