Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams, using an iPad and clinical workflow. Engage one-on-one with patients in their residences to bridge care gaps, while remaining flexible to travel locally and perform virtual visits as needed. Ensure safe performance in diverse home environments that may contain pets or allergens. This part-time role supports Signify Health's mission to provide personalized, compassionate care nationwide, with an anticipated weekly schedule of 29.5 hours.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
