Job Summary
Provide in-home health evaluations to bridge gaps in care and deliver personalized health assessments to patients in their homes. Conduct 45-minute visits including medication and medical history review, physical examination, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, eye exams). Use an iPad and clinical workflow; does not require prescribing, ordering labs, or altering current treatment. Travel locally as needed to visit members in their homes across licensed states; part-time role (~29.5 hours/week) with flexible scheduling and benefits. Must hold an active MD or DO license with the ability to practice in the specified coverage area. This position supports Signify Health under CVS Health, focusing on delivering compassionate, high-quality in-home evaluations and connecting patients with appropriate care. Potential for virtual visits as needed; primary workplace is the patient’s home.
