Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform services such as bone density screening while adhering to strict safety protocols in diverse home environments. Perform virtual visits as needed and maintain flexibility to travel locally across licensed states. Engage one-on-one with patients to bridge care gaps and connect them with appropriate health resources.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
