Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to connect one-on-one with patients in their private residences, reviewing medical history and addressing health questions. Perform virtual visits as needed and provide comprehensive preventive services while adhering to safety protocols in diverse home environments. Travel locally within licensed states to visit millions of people nationwide, bridging care gaps and ensuring PCPs are aware of patient quality of life.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
