Job Summary
In-Home Physician to provide in-home health evaluations for Signify Health, part of CVS Health. Conduct ~45-minute visits with one-on-one patient engagement, including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by the health plan. Use an iPad and a streamlined clinical workflow; not required to prescribe medicines, order labs, or alter current treatment plans. Travel locally within licensed states; virtual visits as needed. Deliver comprehensive diagnostic and preventive services to health-plan members in their homes, contributing to improved health outcomes and care connections. This full-time role offers a benefits package and requires active MD or DO licensure, with multi-state licensure preferred.
Required Qualifications
