Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams, using an iPad and clinical workflow. Engage one-on-one with patients in their private residences to build trusted relationships and bridge care gaps. Perform virtual visits as needed and travel locally within licensed states. Maintain unrestricted medical licenses and adhere to safety protocols in diverse home environments containing potential allergens or pets.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
