Job Summary
Provide in-home health evaluations for patients in their homes, conducting medication/history reviews, physical evaluations, and optional diagnostic tests (spirometry, diabetic eye exams, bone density screening) as ordered by the health plan; use an iPad for the clinical workflow; physicians are not required to prescribe medications or alter current treatments; travel locally to visit patients and connect them with appropriate care; collaborate with Signify Health clinicians to bridge gaps in care; full-time position with comprehensive benefits.
Required Qualifications
- MD or DO license in coverage area
- Active, unrestricted license(s) in coverage area(s)
- Multi-state licenses (preferred)
