Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests such as spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each visit to connect one-on-one with patients in their private residences, reviewing health plans and making referrals to primary care physicians. Perform virtual visits as needed and provide Diagnostic & Preventive Services products while maintaining flexibility for local travel within licensed states. Work full-time hours with a focus on building trusted relationships and improving health outcomes for individuals and families across the community.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Flexible to travel locally and within licensed states, as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
Desired Qualifications
- Multi-state licenses (preferred)
