Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams, using an iPad and clinical workflow. Engage one-on-one with patients in their residences to bridge care gaps, while remaining flexible to travel locally and perform virtual visits as needed. Note that this role does not involve prescribing, ordering labs, or altering treatment regimens. Candidates must be comfortable working in diverse home environments that may include pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
