Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams, using an iPad and clinical workflow. Engage one-on-one with patients in their residences to bridge care gaps, while remaining flexible to travel locally and perform virtual visits as needed. Note that you will not prescribe, order labs, or alter treatment regimens. Candidates must be comfortable working in diverse home environments that may contain pets or allergens.
