Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams, using an iPad and clinical workflow. Engage one-on-one with patients in their residences to bridge care gaps, while remaining flexible for local travel and virtual visits as needed. Perform duties safely in diverse home environments that may contain pets or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
