Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these visits while connecting one-on-one with patients to answer health questions and bridge care gaps. Travel locally within licensed states as needed and perform virtual visits when required. Work part-time for 29.5 weekly hours with flexible scheduling.
