Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Perform virtual visits as needed while using an iPad and clinical workflow to answer patient questions and provide education. Work with Signify Health as a clinician colleague, fulfilling requirements for Diagnostic & Preventive Services products such as bone density screening. Travel locally and within licensed states as needed, adapting to diverse home environments that may contain pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Be flexible to travel locally and within licensed states, as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
