Job Summary
Per Diem Nurse Practitioner or Physician Assistant to conduct in-home health evaluations for Signify Health in Las Vegas, NV. The visit lasts about 45 minutes and includes medical history review, physical examination, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, DEE, bone density screening). Clinicians use an iPad-based workflow and are not required to prescribe medications, order labs, or alter current treatment plans. Responsibilities include visiting members at home, building trusted relationships, and coordinating care to improve health outcomes. Local travel within licensed states is expected. Required qualifications include board certification and an active, unrestricted license in the coverage area; multi-state licenses are preferred. This is a part-time/per diem role with a base hourly rate and incentive structure, plus benefits where applicable.
