Per diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in members' homes, performing 45-minute visits that include health questions, medication/history review, physical evaluation, and optional diagnostic tests per health plan orders. Uses an iPad and clinical workflow; not required to prescribe medicines, order labs, or alter current treatment. Travel locally within licensed states; conducts virtual visits as needed; primary workplace is the member's home; environment may include pets or secondhand smoke. Offers hourly pay with additional incentives per visit; includes malpractice insurance coverage during Signify Health activities and supplies. Requires active NP/PA licensure, board certification, and flexible travel; no relocation required. Per diem position with part-time hours and comprehensive benefits package; eligibility and benefits vary by hours worked.
