Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams, using an iPad clinical workflow. Engage one-on-one with patients in their residences to answer health questions and provide education, while reviewing medical history and connecting individuals to appropriate care. Perform virtual visits as needed and travel locally within licensed states to fulfill service requirements. Note: This part-time role requires 29.5 weekly hours and an active, unrestricted license with board certification.
