Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical exams, and ordered diagnostic tests such as spirometry or diabetic eye exams. Perform virtual visits as needed while utilizing an iPad and clinical workflow. Review patient questions and concerns to guide them toward better self-care without prescribing medication or altering existing treatment regimens. Travel locally within licensed states and work in diverse home environments that may contain pets or allergens. This part-time role supports Signify Health's mission to provide personalized, comprehensive health services nationwide, with an anticipated weekly schedule of 29.5 hours.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
