Utilize an iPad and clinical workflow to perform these visits while engaging one-on-one with patients in their private residences. Fulfill specified Signify Health services including preventive care and education, working as a clinician colleague within a national network. Travel locally and conduct virtual visits as needed, maintaining flexibility to work in diverse home environments. This part-time role requires 29.5 weekly hours and is open to Nurse Practitioners and Physician Assistants with active, unrestricted licenses and board certification.
