Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and additional diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to connect one-on-one with patients, answer health questions, and provide education without prescribing medicine or altering treatment regimens. Travel locally within licensed states and perform virtual visits as needed while maintaining flexibility for diverse home environments. This part-time role supports Signify Health's mission to bridge care gaps by visiting millions of people nationwide to connect them with personalized health solutions.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
