Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Use an iPad-based clinical workflow to connect one-on-one with patients in their private residences to review medical history and answer health questions. Perform virtual visits as needed while traveling locally within licensed states. This part-time role involves working with Signify Health, a national network dedicated to bridging care gaps by providing flexible, personalized evaluations directly to patients' front doors. Candidates must be comfortable performing duties in diverse home environments that may contain pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
