In-Home Nurse Practitioner or Physician Assistant (Part Time) - Honolulu, HI

Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for Signify Health in Honolulu, HI. Visit neighborhoods in patients’ homes for about 45 minutes to conduct medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad-based clinical workflow; not required to prescribe medications, order labs, or alter current treatment. Responsibilities include building trusted relationships to improve health outcomes, visiting people nationwide through Signify Health’s clinician network, and delivering Diagnostic & Preventive Services such as spirometry, bone density screening, and other tests. Flexible to travel locally within licensed states; virtual visits as needed. Primary workplace is the patient’s home, which may include pets or allergens. Qualifications include active, unrestricted licensure, board certification, and multi-state licensure preferred. Part-time benefits available; typical weekly hours ~29.5. Pay range offered is $50.22–$108.15 per hour.
