In-Home Nurse Practitioner or Physician Assistant Part Time - Fallon

Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations for Signify Health, traveling locally within licensed states to visit health plan members in their homes. The role involves a 45-minute visit with a medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by the health plan. Clinicians will use an iPad-based workflow; prescribing medicines, ordering lab tests, or altering the patient’s current treatment plan is not required. Responsibilities include building trusted relationships, providing flexible, comprehensive, and personalized health evaluations delivered to members’ front doors, and conducting virtual visits as needed. Primary workplace is the private residence of health plan members, with the environment potentially varied and including pets or secondhand smoke. This in-home, part-time role is eligible for benefits and requires flexibility for local travel.
