Job Summary
Part-time In-Home Nurse Practitioner or Physician Assistant delivering in-home health evaluations for health-plan members in Erie, PA. Each 45-minute visit includes health questions, medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams, bone density screening) as ordered by the health plan. Clinicians use an iPad and a clinical workflow; they are not required to prescribe medications or alter current treatments. Travel locally to member homes; virtual visits as needed. Must hold an active, unrestricted license (NP or PA), board certification, and preferably multi-state licenses. Anticipated weekly hours ~29.5; pay range $41.85-$90.13 hourly with corresponding annualized salaries." ,
